Phase 1

Provisional Assessment Process (if PAC’d)

Phase 2

Initial Assessment and Carer Orientation

  • Micro modules which are completed in parallel to the assessment process and are introduced by the Assessor.
  • PDF versions that can be printed and taken to interviews to allow those carers who are unable to access online content.
  • Webinar versions for those carers who have challenges in reading writing, webinar versions for those carers who are rural and remote.
  • Fully accessible with multiple modalities

 

Carer Orientation: 5-10 minute videos to accompany PDF handout/resource of the content

Topics include:

  • Introduction content (which is the same as Connecting Kin content)
  • Developing Relationship with the child
  • Developing relationship with the system
  • Role and Responsibilities
  • Requirements such as Centrelink, Blue Card, Medicare, Child Health passport
  • Early days of the placement.
  • Contact
  • Your family

Phase 3

Carer is Approved and introduced to the 12 month Bridging program to Support.

Embedding with LCS Practitioners through the bridging program will include reflective tools, guidance and coaching specific to the monthly topic. Practitioners are to walk alongside the Carer in their 12-month equipping to ensure that the information is embedded in their learning and adjustment to their role.

Phase 5

Collective Conversations Podcast

A podcast specific to the out-of-home care listeners such as Carers, Kinship Carers, Staff and practitioners. A warm and inviting conversation between a facilitator/interviewer and an invited guest with specific expertise or skills in a topic relating to children in out of home care and the care team.
